Home Care Link of CHRC becomes one of the Recognised Service Providers (RSP) of the Pilot Scheme on Community Care Service Voucher for the Elderly(“CCSV”), which offers elderly with daycare and home care services.

 

Service Objectives 

Social workers, nurses and therapists form a multi-disciplinary team to promote the rehabilitation of the elderly through mobilising the community network and encouraging the elderly to go out of the community to enjoy their twilight years. 

 

Service Content 

A wide range of multi-disciplinary home care and full-time or half-time day centre services are provided with flexibility according to the individual health and family conditions of the elderly persons.  

 

Elderly people and carers can enjoy preferential access to the full range of health and elderly services of the organisation, thus achieving "home-community" care and enhancing the quality of life of the elderly and carers.

Service Charges 

-Any value between the highest and lowest community voucher value. 

There are six co-payment levels based on the household income of the elderly, and the elderly are required to pay 5-40% of the value of the service package. 

The service voucher value and co-payment amount are determined by the Social Welfare Department.  

 

Service Application and Enquiry 

The Social Welfare Department will issue invitation letters to eligible elders on the Central Waiting List for Long-term Care Services through the responsible social workers to invite them to join the programme. Enquiries can be made to the social worker in charge. 

 

Withdrawal Procedures 

If an applicant wishes to withdraw from the service, he/she is required to complete the withdrawal procedure by phone or in writing as soon as possible. There is no refund of the remaining monthly balance of the Elderly Community Care Service Voucher.

Home care: 8:30 a.m. to 6:00 p.m.
