Service Objectives
We work closely with the Department of Medicine of Yan Chai Hospital to provide transitional care, rehabilitation and home support services to the newly discharged elderly.
The service provides practical support to the elderly with a view to minimising unnecessary hospital readmission, and provides skills training to the carers to reduce the stress of caring for the elderly.
Service Targets
Elderly dischargees aged 60 or above referred by the Department of Medicine of Yan Chai Hospital.

Service Period
Average duration of service: 6-8 weeks
Service Charges
Service charges are determined in accordance with the standards set by the Social Welfare Department (SWD).
The fees are adjusted according to the recommendations of the Social Welfare Department and depend on the financial situation of the applicants.
Application of the Service
Medical staff of the Department of Medicine of Yan Chai Hospital will refer the needy elders to our team for service.
Withdrawal from Service
Applicants who wish to withdraw from the service are required to notify us in writing or by phone as soon as possible to complete the withdrawal procedure.

Our team consists of social workers, nurses, physiotherapists, occupational therapists and trained carers.
