Service Objectives
Through multi-disciplinary collaboration and an integrated service model, the Centre is a social service unit integrating the four major areas of "medical", "social", "training" and "scientific research". We provide immediate and targeted rehabilitation treatment and support services to people with rehabilitation needs and their families, and link up with social resources and enthusiasts to promote community care. At the same time, we are committed to contributing to the community through our academic, research and human resource development missions.

Home Care and Nursing Rehabilitation Services
Home Care Link - Home-based Rehabilitation Programme
A multi-disciplinary team of healthcare and rehabilitation professionals will provide comprehensive home care and rehabilitation support services to people who are injured or frail, chronically ill, or have rehabilitation needs.
60-Day Stroke - Home-based Rehabilitation Programme
A personalised and comprehensive rehabilitation programme designed for each stroke survivor, comprising physiotherapist-designed therapeutic techniques and intensive home-based rehabilitation programmes provided by stroke survivors/assistant health workers, to help them grasp the golden recovery period of stroke and to strengthen the rehabilitation performance thereafter.
Day Rehabilitation and Health Promotion Services
Day Rehabilitation Centres
Provide immediate and targeted rehabilitation treatment and community living support services, including full-time or half-time and holiday day rehabilitation centre services, and arrange day respite services for the disabled or infirm to give carers a break.
Carer and Health Education Support Centre
Provide caring skills training and support activities for carers.
To organise community health promotion education and training programmes to promote the health care competence of community members.
